Inflammatory bowel disease (IBD)

小生にとって炎症性腸疾患は大変懐かしい病気のひとつだ。医師国家試験に合格して、消化器外科に入局し最初に担当したのがクローン病を抱える若い女性の患者さんだった。彼女が入院した理由は、瘻孔形成による消化管穿孔。当時はペンタサといった特効薬もなかった時代。時が流れて、今やサラゾピリンやペンタサ、ステロイドの服用によって、合併症の発症率はかなり減ったようだ。小生にとっては大変思い入れのある消化器疾患のひとつだ。 

さて、当論文の内容は素人が理解するには相当難しい。ハードルはかなり高いが、抄録を見る限り、理解すべき量は大したことはなさそうだ。元となる論文を見つけたので一部を転載し、内容を検討してみたい。さらにCEACAMに関する論文もあったのでそちらも参考になる。要は下に掲げた新聞記事のような簡単な理解で十分だと思う。

クローン病や潰瘍性大腸炎 難病防ぐ腸内物質確認 神戸大  

 激しい腹痛や下痢などを伴う難病のクローン病や潰瘍性大腸炎の発症を防ぐ鍵となるタンパク質を、神戸大の研究グループが世界で初めて確認した。このタンパク質が発症を抑止する仕組みの一端も解明。予防薬や治療薬開発への貢献が期待できるという。研究成果は米科学アカデミー紀要電子版に掲載される。(藤森恵一郎)

同大大学院医学研究科の的崎尚(たかし)教授と村田陽二准教授らの研究グループの成果。

 クローン病や潰瘍性大腸炎は、腸の粘膜に慢性的な炎症や潰瘍が生じる原因不明の病気で、ともに医療費助成対象の「指定難病」。国内では20~30代を中心に発症し、患者数は合わせて約20万人に上るとされる。炎症を抑える薬はあるが、根本的な治療薬はまだ開発されていない。

 的崎教授らは以前、腸の内側を覆う腸上皮細胞の表面にある「微絨毛(じゅうもう)」という細かな突起から「SAP-1」というタンパク質を発見していた。今回、その機能を調べるため、マウスの腸上皮細胞からSAP-1を除去したところ、腸炎の発症率と重症度が著しく高まることを確認した。

 さらに、SAP-1が、同じく微絨毛にある「CEACAM20」というタンパク質の機能を抑えることで、腸炎の発症を防いでいる可能性が高いことも判明したという。

 研究を主に進めた村田准教授は「SAP-1やCEACAM20の機能を制御する化合物の考案が、新規の薬の開発につながると期待している。創薬に向けた研究を進めたい」としている。

Protein tyrosine phosphatase SAP-1 protects against colitis through regulation of CEACAM20 in the intestinal epithelium

Significance

Much attention has been recently paid to the role of intestinal epithelial cells in the homeostatic regulation of intestinal immunity. Here we show that ablation of stomach-cancer–associated protein tyrosine phosphatase 1 (SAP-1) markedly increased the severity of colitis in interleukin (IL)-10–deficient mice, suggesting that SAP-1 protects against colitis in a cooperative manner with IL-10. We also identify carcinoembryonic antigen-related cell adhesion molecule (CEACAM) 20, an intestinal microvilli-specific membrane protein, as a dephosphorylation target for SAP-1. Indeed, tyrosine phosphorylation of CEACAM20 promotes the binding of spleen tyrosine kinase (Syk) and activation of nuclear factor-κB (NF-κB), thereby inducing production of chemokines such as IL-8. Thus, we propose a mechanism by SAP-1 and CEACAM20 in the intestinal epithelium for regulation of the intestinal immunity.

腸粘膜上皮の免疫学的意義は何か。

IL-10欠損マウスにおいて、SAP-1(胃癌関連蛋白チロシンホスファターゼ)の欠落により大腸炎が重篤化した。SAP-1は、IL-10ととともに働き、大腸炎から個体を防護することを示唆する。一方、CEACAM20(CEA関連細胞接着分子)は、SAP-1の脱リン酸化のターゲットであることが分かった。事実、CEACAM20のチロシンリン酸化は、Sykの結合とNF-κBの活性化を促進する。これにより、IL-8などのケモカインの産生が誘導される。つまり、腸上皮の腸免疫の制御におけるSAP-1とCEACAM20の関与が推定される。 

Abstract

Intestinal epithelial cells contribute to regulation of intestinal immunity in mammals, but the detailed molecular mechanisms of such regulation have remained largely unknown. Stomach-cancer–associated protein tyrosine phosphatase 1 (SAP-1, also known as PTPRH) is a receptor-type protein tyrosine phosphatase that is localized specifically at microvilli of the brush border in gastrointestinal epithelial cells. Here we show that SAP-1 ablation in interleukin (IL)-10–deficient mice, a model of inflammatory bowel disease, resulted in a marked increase in the severity of colitis in association with up-regulation of mRNAs for various cytokines and chemokines in the colon. Tyrosine phosphorylation of carcinoembryonic antigen-related cell adhesion molecule (CEACAM) 20, an intestinal microvillus-specific transmembrane protein of the Ig superfamily, was greatly increased in the intestinal epithelium of the SAP-1–deficient animals, suggesting that this protein is a substrate for SAP-1. Tyrosine phosphorylation of CEACAM20 by the protein tyrosine kinase c-Src and the consequent association of CEACAM20 with spleen tyrosine kinase (Syk) promoted the production of IL-8 in cultured cells through the activation of nuclear factor-κB (NF-κB). In addition, SAP-1 and CEACAM20 were found to form a complex through interaction of their ectodomains. SAP-1 and CEACAM20 thus constitute a regulatory system through which the intestinal epithelium contributes to intestinal immunity.

腸免疫の制御機序はいまだ不明である。SAP-1は、レセプター型のチロシンホスファターゼであり、胃腸粘膜上皮絨毛に局在する。炎症性腸疾患モデルであるIL-10欠損マウスにおいて、SAP-1を欠落させると、大腸におけるサイトカインやケモカインの産生が上昇し、大腸炎が重篤化した。SAP-1欠落マウスにおいて、CEACAM2(腸絨毛特異的細胞膜貫通蛋白ファミリーのひとつ)の産生が著明に上昇した。CEACAM20のチロシンリン酸化は、Sykの結合とNF-κBの活性化を促進し、IL-8などのケモカインの産生が誘導された。さらに、SAP-1とCEACAM20は、それぞれの細胞外ドメインの相互作用により、蛋白複合体を形成することが分かった。かくして、この2つの蛋白は腸免疫の制御機構に重要な役割を果たしている可能性が高い。

Signaling by epithelial members of the CEACAM family – mucosal docking sites for pathogenic bacteria

Abstract

Carcinoembryonic antigen-related cell adhesion molecules (CEACAMs) comprise a group of immunoglobulin-related vertebrate glycoproteins. Several family members, including CEACAM1, CEA, and CEACAM6, are found on epithelial tissues throughout the human body. As they modulate diverse cellular functions, their signaling capacity is in the focus of current research. In this review we will summarize the knowledge about common signaling processes initiated by epithelial CEACAMs and suggest a model of signal transduction by CEACAM family members lacking significant cytoplasmic domains. As pathogenic and non-pathogenic bacteria exploit these receptors during mucosal colonization, we try to highlight the connection between CEACAMs, microbes, and cellular responses. Special emphasis in this context is placed on the functional interplay between CEACAMs and integrins that influences matrix adhesion of epithelial cells. The cooperation between these two receptor families provides an intriguing example of the fine tuning of cellular responses and their manipulation by specialized microorganisms.
